Community Health Needs Assessment

CarePartners defines its primary service community as Buncombe County and commits resources to its identified health needs. CarePartners partners with Buncombe County Department of Health and WNC Healthy Impact to conduct a Community Health Need Assessment (CHNA) in Buncombe County every three years to determine community health needs. The most recent CHNA was completed on December 3, 2012.

This document outlines plans for CarePartners to support specific community benefit efforts as part of a larger community-wide plan.
