CarePartners Adult Day Services has grown from a small program in the basement of a church to one with the capacity to serve 70 adults on any given day. We invite you to tour our 7,500 square-foot secured facility, featuring specialized areas and programs where adults with all levels of cognitive/functional ability and health needs can enjoy a safe, stimulating environment.
Located on the campus of CarePartners Health Services and renovated through grants from CarePartners, Janirve and the Kate B. Reynolds foundations, CarePartners Adult Day Services offers easy access to speech, occupational and physical therapies from CarePartners professional staff. CarePartners Adult Day Services is an award-winning adult day facility recognized by the N.C. Adult Day Services Association.
Our Participants
While many of the participants have some form of dementia, individuals with a wide variety of impairments enjoy our safe, yet stimulating daytime environment. The number of days a participant attends is based on the needs of the individual and the caregiver, as well as days available at the center.
CarePartners Adult Day participants may also access the CarePartners Geriatric Care Management Coordinator, who can help coordinate care for individuals, ensure appropriateness of services and unburden family members not familiar with the resources.

Care Provided
CarePartners Adult Day Services offers participants a wide variety of services by qualified personnel:
- Safe and Stimulating Activities
(crafts, travelogues, music, fitness, pet visits, seasonal celebrations and more!)
- Personal Care Assistance
(toileting, showers and hair care)
- Administration of Medication
- Monitoring of Vital Signs
- Monitoring of Blood Glucose Readings
- Daily Meals
(breakfast, lunch and afternoon snacks)
- Attention to Dietary Needs
(tube feedings, nutritional supplements, vegetarian and diabetic diets)
- Caregiver Education and Support Groups
- Fenced in garden area
- Quiet area with recliners
- Geriatric Care Management (Private Pay)

Initiating Services
There is no formal referral required and tours are available. Here are the steps for initiating services:
- Step 1: Request necessary forms, including medical form to be completed by participant's physician prior to first day visit
- Step 2: Schedule one-day, four hour visit
- Step 3: Arrange participant's schedule and complete remaining forms
- Step 4: Participant begins program

What Are the Fees?
Qualifying veterans are covered through a contract with the Veterans Administration.
Medicaid coverage includes adult day services for those qualified through the CAP program.
Discounted rates and financial assistance are available to those who qualify (completion of a Financial Disclosure Form required).
Private pay charges are based on a level of care fee structure. An assessment tool is used to determine the level of care required while at CarePartners Adult Day Services.

Transportation
Public transportation may be available through Mountain Mobility. It is based on individual need and available space. An additional charge may be necessary for those in need of transportation.

Certification
CarePartners Adult Day Services is a nonprofit agency that provides high quality day care and day health services for impaired adults. CarePartners Adult Day Services receives United Way funding, which provides for quality care at reasonable rates. CarePartners Adult Day Services is certified by the North Carolina Division of Aging and is recognized by the North Carolina Adult Day Services Association.
