Headquartered in Asheville and with branch offices in Hendersonville and Waynesville, CarePartners Home Health Services works in close collaboration with area hospitals and physicians to assure that the transition to homecare is a smooth one for both patient and family.
 
 
Nursing Care
 
Patients who need nursing care are assigned a primary CarePartners Home Health nurse who is responsible for their care. The primary nurse assesses the patient's progress, offers instruction on important healthcare issues, and works with the patient's physician to provide the proper treatment at home. Other services CarePartners Home Health nurses provide include:

Specialty Care
 
Under the direction of specialty trained nurse clinicians, CarePartners Home Health Services offers a number of specialty care services, including:
  • Cancer Care
    chemotherapy, pain & symptom control, ostomy care, post-mastectomy exercises & respite care
     
  • Cardiac Care
    heart & lung assessment, post-surgical follow-up & lifestyle instruction
     
  • Pediatric Care
    apnea monitoring, phototherapy, nebulizer therapy & care for medically fragile children through the Circle of Care program
     
  • Wound & Skin Care
    with Western North Carolina's first homecare-based Certified Enterstomal Nurse (CETN)
     
  • Diabetic Care
    diabetic teaching program & assistance in control measures to maximize the patient's health
     
  • CrossRoads
    care for patients diagnosed with terminal illness who are not ready for hospice care
     
  • Home Telehealth
    daily monitoring of blood pressure, pulse, oxygen saturation, weight and temperature
     
  • Psychiatric care
    a home care program provided by experienced, psychiatric nurses in order to maximize the patient's potential to live at home

Rehabilitation Therapy
 
A variety of home rehabilitation therapies are available through CarePartners Home Health Services to help patients recover faster and regain independence. Our team of rehabilitation specialists include:
  • Speech Therapists
    who assist the patient in regaining speech, communication & swallowing functions
     
  • Physical Therapists
    who develop specialized programs aimed at increasing the patient's performance and mobility
     
  • Occupational Therapists
    who assist the patient in regaining self-care activities & independence

Social Work Services
 
Sometimes illness or injury force changes in patient/family routines and finances. CarePartners Home Health social workers are available and offer assistance in adjusting to the changes and in accessing community services and appropriate financial aid when needed.
