Is CarePartners Rehabilitation Hospital right for you or your loved one? Your doctor, discharge planner, or a CarePartners nurse can help to make this determination, but here are some important guidelines.
Are you unable to perform at least two everyday activities?
Adults and teenagers whose illness or injury prevents them from being able to perform at least two everyday activities, such as walking, eating, speaking, or self-care, may be able to benefit from the services of CarePartners Rehabilitation Hospital.
Also, to be admitted, patients must:

Also, to be admitted, patients must:
- Be able to learn new information
- Have potential for significant improvement
- Be able to tolerate at least three hours of intensive rehabilitation activities per day
- Require 24-hour-a-day nursing and physician availability
- Need more services than those that can be provided by a skilled nursing facility
- Expect to be discharged in a timely manner once their rehabilitation goals
have been met.
