The Cardio/Pulmonary Program at CarePartners aims to relieve patients and their families of that anxiety.

A nurse care manager and an interdisciplinary team of experts specifically trained in cardiac and pulmonary related problems work with you and your doctor (as well as the staff of the hospital, if you’ve just been discharged) to determine how you could benefit from our Cardio/Pulmonary Program.

Services are available in the following settings:
- Outpatient rehabilitation: Regain strength and endurance in one of our conveniently located outpatient clinics
- Cardiac care in your home: Our home health experts can monitor your health and provide education and instruction on lifestyle choices to help manage and even improve your condition. Private duty skilled nursing is also available, from two hours up to full-time, 24-hour care.
- Home telehealth: This technology allows you, with a device in your home, to send important information about your health to nurses in our CarePartners offices. The HomeMed Health Monitoring System is easy to use and allows us to monitor your blood pressure, pulse, weight, temperature, and other vital statistics.
- Homemaker and personal care services: For assistance at home, such as bathing, dressing, meal planning and preparation, and errands.
- Adult day services: For individuals who need a safe place to be during the day while caregivers are at work or taking care of personal needs; includes a nurse on staff to monitor health and administer medications.
- Hospice and palliative care: Comfort care for those with a life-limiting illness.
