This is the experience everyone should have at the end of life. It’s what hospice care is all about. When you or someone you love is facing a terminal illness, CarePartners Hospice Services is here to provide physical, emotional and spiritual support.

Hospice care providers work to improve the quality of our patients’ lives, helping them to live their last days, weeks or months as free of pain and other distressing symptoms as possible.

Care in the Place that You Call Home

Most of our patients receive hospice care in the place they call home – their own home, a nursing home or an assisted living facility.

For those patients who are admitted to the Hospice program but cannot remain at home, the CarePartners Hospice Solace Center is an adult and pediatric inpatient facility on our main campus.

Continued Support for Loved Ones

Support for grieving family and friends is provided as part of the CarePartners Hospice program. Services include professional grief counseling, grief education classes, support groups for adults and children, and memorial services.

Making the Decision to Enter Hospice Care

At CarePartners Hospice we offer care to all ages, from the very young to the oldest members of our community. In order to be eligible for hospice care, a patient must have a terminal diagnosis with a life expectancy of months rather than years. Palliative care is also available for patients who are not yet ready for hospice. You can read more about eligibility for hospice care here.
